Chronic Care Management via a Digital First Strategy

As the global medical community began to realize the long-term health impacts of COVID, Virtua Health was quick to respond by launching a telemedicine service for patients with post-acute sequelae of COVID. Seven hundred and fifty patients later, Virtua Health has developed a successful and scalable model for managing chronic conditions via telemedicine and are excited to share their lessons learned.
