Best Practice in Integrated Care - What does success look like?

Integrated care has long been seen as the Holy Grail of improved connectedness of care and a means to providing better health outcomes for all. With more patient information and data available than ever before, integrated systems certainly play a crucial part in the day-to-day life of healthcare organisations, but are they really delivering on their promise? In this first session of the Silos to Integrated Care track, experts take a step back to examine best practice in integrated care and ask what can really make it successful? The session will explore opinions from global, national, and regional perspectives, with experts discussing the challenges of integrating care within health systems, and the impact this can have on safety, outcomes, creating more efficient systems and delivering patient-centred care.
