Before we can fulfil the potential of digital and data to transform health services and connect patients and providers, there are complex challenges to overcome around interoperability, organisational silos, trust, and governance. This session will take a deep dive into integrated care from a practical perspective, including how to get started and what the basic requirements needed for successful implementation are. We will also discuss the benefits of a connected electronic medical record (EMR) and its role in supporting better integrated care across sectors, between healthcare providers and between patients and healthcare professionals. Experts will explore how alliances can support better integrated care by using innovative private partnerships in collaboration with public services. We will also take a look at common blockers, limitations and challenges to integrated care and how to overcome them, highlighting inspiring  examples of successful projects that have overcome these issues.
